HELP Hawaiian feather lost it head twice

I have I feather duster for four weeks and all was good woke up and found his head had come off. I did not panic I waited a week and saw that it was growing a new head. After a week or so of growing his new head today it lost it again. There is some pulsating zinnia that was getting close to it and a flower pot that is about 5 inches from the feather. Could one of these be bothering it or should I be looking at the fish being a pest? Water changes of done regularly and water is in spec. Nothing else seems to be having problems. Fish:chromisis, blennie, dimond goby, yellow goby, citron goby, firefish, coco worm, maxium clam. Corals:flowerpot, leather, acropora, ricorria, duncan, zoas, frogspawn.
 
Normally I would say its not a big deal if its new in the tank but since its happened twice in a shor period of time there may be a problem. If any of the corals are able to come in contact with it then I would try and move something around to prevent that from happening. I have a blennie that lives in the large barnacle shell I put three Hawaiian feathers in and he does irritate them but none have lost a head in 5 or 6 months.
 
I have moved it to the other side of the tank and the worm is coming out of his tube about 1 inch. I have check the water pars and nit=0 phos=0 cal=420 I hope I don't stress it too bad and it can survive.
 
xenia, star pollup, zoas all give off noxios irritation stuff. Also under fed feathers will drop heads. Charcoal or chemi pure can help keep things in check.
 
I've never had any luck with feathers. They seemed to be doing great and then a few weeks later...what you described. No corals were touching mine. After two tries I decided to stop killing them and wasting my money. The only other casualties I had were due to my mean true perc (he got along with seahorse though) who's been there since day one. (Seahorse (three of them) died because I was on travel and wife 'forgot' to feed them).
