mystery wrasse has needles coming out of its face

I recently noticed that my mystery wrasse has some sort of clear needle-like object coming out of its nose and the mouth of the fish it constantly open. Its almost like it is choking. Can anybody tell me what the issue might be with my fish?
 
A picture is needed for this one. In addition to a clear photo, I'd like to know all the other marine life forms in the tank. Do you use live rock?

How long have you had this fish?

How long has your tank been up and running since it cycled?
 
I’ll try to make some pictures today- how do you post it? I had privies 55 gal tank with 55 lb of live rock running for 3 years, and I just put all of it in this 45 gal about month ago. I have one Yellowtail Damselfish, two Blue Sapphire Damselfish, one Green Mandarin, two Ocellaris Clownfish one Algae Blenny, Tiger-striped Serpent Sea Star, Caribbean Serpent Sea Star, Crocea Clam, Pink Bulb Anemone, Emerald Mithrax Crab, Cleanup Crew and Berghia Nudibranchs.
 
Then i'd guess it tried to grab a bristle worm and got stung. Sounds pretty normal to me.
