Yellow Tangs with chewed fins

I have 4 YT they all seem to have chewed fins.

There is one Damsel and a few brittle stars in 215g with them. (reef)

It's usually one of three things, or a combination of any of the three things:

1. Low or incorrect nutrition;
2. Tank mates picking on fish; and
3. Disease or infection (including water quality poisoning).
